PROGRAM AND LOCATION: VOA-NCNN, a Certified Great Place to Work, is recruiting for a full-time, Permanent Supportive Housing (PSH) Case Manager to anchor on-site services at Pacific Crest Commons, a new housing community in Truckee, CA. You will support tenants in 10 PSH units funded through No Place Like Home (NPLH). Tenants are adults living with serious mental illness who have experienced homelessness, including chronic homelessness, or are at risk of it. The program is part of VOA's services in the North Lake Tahoe–Truckee region, alongside our Pilot Navigation Center, and is delivered in partnership with Nevada County Behavioral Health (NCBH). This role follows Housing First principles. Housing is never conditioned on treatment, and every service is voluntary. Your success is measured by one thing above all: tenants keeping their homes, with a target of 85% or more still housed at 12 months. You'll create and maintain an environment where dignity thrives alongside structure, where participants feel seen as whole people rather than problems to solve. The work schedule is Monday - Friday 8:30AM - 5:00PM.

Responsibilities

POSITION SUMMARY:

 

This position is responsible for the development of a case management plan to assist PSH participants in developing a greater degree of self-reliance and independence. It will include linking participants to education, employment, budgeting strategies/ money management and addressing any bio, psycho, social issues that may lead to homelessness, criminal behavior, or otherwise non-productive lifestyles. The Case Manager will create and maintain an environment where dignity thrives alongside structure, where participants feel seen as whole people rather than problems to solve. This role follows Housing First principles. Housing is never conditioned on treatment, and every service is voluntary. Success is measured by one thing above all: tenants keeping their homes, with a target of 85% or more still housed at 12 months.

 

OBJECTIVES/ACTIVITIES:

 

Responsible for performing daily work requirements to achieve established objectives of the department such as

  1. Hold a welcome visit with each new tenant in their first week, then complete a strengths and needs assessment.
  2. Build a written, tenant-driven Individualized Service Plan (ISP) within 30 days of move-in, including a Crisis Response Plan. Review it at least every 90 days.
  3. Carry out weekly in-unit check-ins to track ISP goals and unit condition.
  4. Serve as the central hub of each tenant's care team, coordinating internal and external providers.
  5. Use Motivational Interviewing and strength-based, trauma-informed approaches; arrange interpreters as needed.
  6. Maintain positive relationships with program participants, landlords, Volunteers of America staff and all community service agencies.
  7. Create and maintain case management files on each participant which will include personal information, educational and job skills, short-term and long-term plans to become self-sufficient in all areas of his/her life and documentation of participants’ progress and assistance in removing obstacles to set goals.
  8. Provide evaluations of individual clients’ needs to maintain their housing. Document all follow-up and placements.
  9. Provide crisis management as needed and mediate conflicts with fellow participants, community partners and neighbors.
  10. Complete all discharge paperwork in a timely fashion.
  11. Complete budget and housing stabilization plans with participants.
  12. Provide resource information and appropriate case planning specific to any participant who has physical or mental impairment.
  13. Participate in Volunteers of America and outside training sessions.
  14. Perform other reasonably related duties as assigned by the Program Manager.
  15. Conduct monthly house meetings
  16. Arrange of facilitate participant transportation, when needed.

Responsible for assistance with client development, tenancy support, and prevention of protection.  

  1. Liaison between clients and community service agencies to meet client needs.
  2. Maintain a list of all available community services and providers.
  3. Assure a safe, non-judgmental environment for the clients.
  4. Maintain positive, professional relationships with clients and staff.
  5. Participate in agency and outside training sessions.
  6. Track and monitor signs of housing instability (late rent, lease violations, neighbor relations, unit condition) and conduct intervention early.
  7. Coach tenants on budgeting, lease terms, unit upkeep, daily routines, meal planning and hygiene, at their own pace.
  8. Help with move-in (utilities, household supplies, orientation), lease renewals and reasonable accommodation requests.
  9. Respond within 1-3 business days to any lease violation or eviction notice with a written stabilization plan.
  10. Meet with property management at least monthly and mediate landlord–tenant and tenant–tenant conflicts.

 Record keeping and communication.

  1. Maintain files on each participant short- and long-term plans to maintain and improve self-sufficiency.  Document progress and assistance in removing obstacles to set goals.  Provide routine documentation of coordination and follow-up of all areas and collecting financial information. Keep records confidential under HIPAA and 42 CFR Part 2.
  2. Maintain participant data in HMIS, Coordinated Entry and Credible. Document services and outcomes for County reporting
  3. Provide records and services in compliance with agency policies, regulatory and funding source requirements.
  4. Maintain daily, clear and thorough communication with the Program Manager on all areas of responsibility listed in this job description.

 Responsible for all areas of daily program maintenance in compliance with company policies.

  1. Document and report all client concerns and/or infractions that impact program security.
  2. Secure approval for exceptions to standard policy.
  3. Notify management of potential workflow problems and resource needs necessary to attain performance standards.
  4. Adhere to staffing schedules to provide adequate/safe staffing coverage and to accomplish program objectives.
  5. Provide emergency shift coverage as needed.
  6. Perform reasonably related duties as assigned by the Program Manager.

 Behavioral health, medical and benefits linkage

  1. Make warm referrals to NCBH therapy, case management and psychiatry; offer ongoing, low-pressure invitations to tenants not yet engaged.
  2. Support appointments with scheduling, reminders, transportation and follow-up; join NCBH care coordination meetings.
  3. Screen tenants for primary care at move-in and link them to local providers, dental, vision and specialty care; coordinate CalAIM Enhanced Care Management where eligible.
  4. Complete a benefits screening within 30 days of move-in and assist with SSI/SSDI, Medi-Cal, CalFresh, IHSS, Veterans' benefits and representative payee options.
  5. Maintain a current file of Truckee-area resources available year-round.
  6. Work to develop and maintain strong ties with NCBH, Nevada County HHSA and local providers

Qualifications

EDUCATION AND EXPERIENCE:

A bachelor’s degree in social work or a related field is preferred. A minimum of two (2) years’ social work-related education from an accredited academic institution or (3) years of experience working with the homeless population or alcohol/drug recovery programs is required, or a combination thereof. This position requires demonstrated ability in record keeping, proficient written and verbal communication skills, and knowledge of problems unique to women, minorities, persons with disabilities, persons with a history of substance use, individuals with low to no income, and general unhoused population. This position requires CPR and First Aid training within ninety (90) days of employment date and recertification as necessary. A valid driver’s license, reliable transportation and ability to meet organizations insurance carrier guidelines required.
